The Shelby County Health Department will be sponsoring a Physical Clinic on Thursday, June 17, 2021.
WHERE & WHEN:
High School Cafeteria beginning at 9:00am (for the initial exam) (weight, height, blood pressure, etc.). Students will need to return at 10:30am for the final visit with the doctor.
Students must have a parent/guardian sign the physical before the exam will be complete. Forms are available in the main foyer of Shelbyville High School.
Fee: 20.00
FREE with Proof of Medicaid Card
School PHYSICALS are REQUIRED FOR:
students entering Pre-K and Kindergarten
students entering 6th and 9th grades
students enrolling in an Illinois school for the first time
+ all students who will be competing in Sports for the 2021-2022 school year
